• Personal last 6 months, Maximum Amount, and the latest updated bank statement with the bank seal and signature on every page 
• Last 3 years Income Tax Papers (Acknowledgement Page)
• Investment papers – FDS/PPF/Shares/Mutual Fund
• Property Proof – If any
• For Student - School/College ID or Bonafide Certificate.
• Children Birth Certificate is mandatory(Below 18 years)
• If Retired – Retirement Proof

If Working:

• Leave Letter from Company with Name, Designation, Leave Date, Purpose of travel, and the signing authority person's Name, designation with company seal must.
• Last 3 months’ salary slips with company seal & Signature must.

If Business:

• Covering Letter on Business Letter Head with Company Seal
• The company's last and latest updated 6-month  bank statement
• Company Income Tax paper last 03 years(Acknowledgement Page)
• Business proof 

If Visiting Relatives:

• If visiting relatives along with their Invitation Letter, passport/ PR copy, Bank statement, Accommodation Proof, IT returns

In Case Children Traveling:

• If Children travel alone or with one Parent, then a NOC letter from the other Parent or both parents is required with passport copies. (On Rs.100/-  Stamp paper & Notarized)
• In  case Children traveling with one Parent require Form No. 1229
• In  case Children traveling with other parents/relatives require Form No.1257

Important Note:  

• Health Insurance is mandatory if the applicant age above 70 years
• All Applicant above 75 Years has to go for a medical check-up
• For a Long Term Visa, mention the proper Travel date with reason in the Covering Letter and the Detail Form                    
• If Application Process for the 6-month & 12 Month each and every applicant has to go for a medical checkup.
• Medical Check up Charges of Rs. 10,000/- Per Person Approx carry at the time of medical check up.
